Husband's First Name: Enter the husband's first name as he customarily uses it for signing all formal documents. This will be the name displayed in your living trust agreement.
Husband's Last Name: Enter the husband's last name or surname as he customarily uses it for signing all formal documents. This will be the name displayed in your living trust agreement.
Wife's First Name: Enter the wife's first name as she customarily uses it for signing all formal documents. This will be the name displayed in your living trust agreement.
Wife's Last Name: Enter the wife's last name or surname as she customarily uses it for signing all formal documents. This will be the name displayed in your living trust agreement.
Street Address: Enter your residential street address.
City: Enter your city or town of residence.
County: Enter your county of residence.
State: Select your state of residence. As state laws will determine the laws which apply to the management and operation of your living trust, it is important to ensure that you select your correct state of residence.
Name of Trust: Select a name for your living trust. For example, "John Smith and Mary Smith Revocable Living Trust". This will appear as "the John Smith and Mary Smith Revocable Living Trust" in your living trust document. Do not use comas in the trust name.
