Last Name: Enter your last name or surname as you customarily use it for signing all formal documents. This will be the name displayed in your deed of assignment.
Street Address: Enter your residential street address.
City: Enter your city or town of residence.
County: Enter your county of residence.
State: Enter your state of residence. As state laws will determine the laws which apply to the transfer of assets to your living trust, it is important to ensure that you select your correct state of residence.
Name of Trust: Enter the precise name of your living trust as set out in your revocable living trust agreement.
Date of Trust: Select the date on which your revocable living trust was created. This will be the date set out in your revocable living trust agreement.
